Yes, you pay once to buy the game
(either digitally or in a normal box) then you must pay a subscription which can
be paid at different intervals (and resultant levels of discount).
How much are the subscriptions?

$12.95 per Month
SOE All Access Plan (EQ is included) - $21.99 per Month
$35.85 for 3 Months
$65.70 for 6 Months
$119.95 for 12 Months (a 23% discount off the monthly rate).
$199.95 for 24 Months (a 35% discount off the monthly rate).
All pricing plans are recurring,
meaning you will continue to be billed at the appropriate interval, until you
cancel your subscription. Note that purchasers of the full game will receive
their free month of game play after supplying valid billing information. If you
cancel your subscription during the free month, no charges will be made to your
credit card.
What is a gamecard?
A gamecard is a card sold in some
stores that allow you to subscribe to EverQuest for a limited period without
using a credit card. These are rare outside the United States and support seems
to be dropping for them.
